*request for recordal of goods in Ukrainian language in the form determined by the Rule of the Cabinet of Ministers of Ukraine of April 28, 2001, ¹ 412 "About Approval of the Provisions for the Order of Registration and Transfer of Goods Having Intellectual Property Subjects through the Customs Border of Ukraine";
*extract from corresponding register of the state authority that issued the document of protection as to the registration of intellectual property rights in Ukraine, said document evidencing that the statement of registration is valid by the moment of filing the application;
*detailed description of the intellectual property subject enabling customs office to identify it and its code according th the Ukrainian Classification of the goods of foreign-economic activity (UKG FEA);
*specimens of the intellectual property subject or the photo images;
other information as to intellectual property subject giving the possibility to identify it;
information about the countries, manufacturers and methods of transportation of infringing goods, if any;
letter of guarantee (obligation) to compensate spending for storing, examination of intellectual property subject and other spending of customs authority as well to compensate damages of the goods owner in case of non-confirming infringement;
*document confirming official fee payment;
